Haven Crest Homes is a Wisconsin Medicaid-enrolled provider serving Family Care members, IRIS participants and private-pay families in all 72 counties. Our home base is Superior, with teams rooted in Douglas and St. Croix counties, and we build local DSP teams wherever members need us, including your own preferred caregiver.
Start times vary by area. Call us and we'll tell you plainly how quickly we can begin in your community.
Every inquiry and every referral gets a response. No voicemail black holes.
A supervisor visits your home within the first month of service, then checks in every month.
A change in health, mood, mobility or safety is reported to your care team the day we notice it.
A second DSP who has already met you, so a sick day never means a stranger at the door.
Call or send a request. We set up a short virtual meeting with you, and family or your care manager if you'd like them there, to learn what a typical day looks like and what has become difficult.
We write a Service Plan with you: which tasks, which days, which preferences. If you have a care manager or IRIS consultant, we coordinate authorized hours with them before anything starts.
You're matched with a DSP trained on your plan, or with your own preferred caregiver whom we've hired and trained. Every visit is documented.
A Haven Crest supervisor visits your home within the first 30 days of service to confirm the support is right, then keeps checking in on a regular schedule.
In-home support like ours is covered for many Wisconsin residents through Medicaid long-term care programs. Not sure where you stand? Call us and we'll walk through it together.
Family Care is Wisconsin's managed long-term care program for frail elders and adults with disabilities. A care manager at your managed care organization (MCO) authorizes the services in your plan. Ask them to authorize Haven Crest Homes.
Family Care MCOs serving Douglas and St. Croix counties:
IRIS (Include, Respect, I Self-Direct) lets you manage your own budget and choose your providers. Your IRIS consultant can help add Haven Crest Homes to your plan.
For families who want support now, or need hours beyond what a program covers, we offer hourly private-pay care with the same trained staff, the same written plan and the same supervision.
Private-pay options and free consultation →Your county Aging & Disability Resource Center (ADRC) does the eligibility screen at no cost and explains your options. We're glad to help you make that first call.
Generally no. Medicare and Medicare Advantage plans do not cover ongoing non-medical in-home support. If that's the only coverage you have, call us anyway. Many people qualify for Medicaid long-term care without knowing it, and we'll point you to the right office.
In some situations, yes. Family Care and IRIS allow certain family members to be hired as paid caregivers through an agency, subject to program rules and MCO approval. Haven Crest Homes can employ, train and supervise a family caregiver so the support stays reliable and compliant. See our preferred caregiver program, then ask your care manager.
Many people are best supported by a family member, friend or neighbor who already knows them. If your program allows it, Haven Crest Homes can hire that person, train them, pay them, and manage the plan, so the care you choose is also care that's reliable and compliant.
Paid family and preferred caregivers are permitted under Family Care and IRIS, subject to program rules and your MCO's or IRIS consultant's approval. Some relationships, such as spouses or legal guardians, have limits that depend on the program. We'll tell you plainly what's allowed before anyone's hopes get set.
Private-pay families can use the same arrangement with no program approval needed.

Most families call us at a hard moment: a fall, a hospital discharge, a parent who insists they're fine. Here's what we tell them.
Your DSP arrives on time, helps with washing and dressing the way you like, makes breakfast, and notices the things a checklist wouldn't.
Laundry in, lunch made, a trip to the pharmacy or the library, the dog let out. Independence supported, not replaced.
The visit is documented, your family gets the update they asked for, and anything that changed reaches your care team the same day.
Common signs: missed meals or medications, a decline in hygiene, unopened mail, a recent fall, a caregiver who is exhausted, or a doctor or discharge planner recommending help at home. Support can start small, a few mornings a week, and grow only if needed. Our goal is to keep someone independent, not to take over.
A supervisor comes to the home for about an hour. We walk through a typical day, look at the home together for safety, talk about preferences (how coffee is taken matters), and write a Service Plan. Family is welcome in the conversation when the member wants them there.
That's the goal, and it's why we invest in keeping our DSPs. You'll have a primary DSP trained on the plan and a named backup who has also met the member, so a sick day doesn't mean a stranger at the door.
Every visit is documented. With the member's permission, we share updates with family on the schedule you prefer: a quick text after visits, a weekly summary, or a call when something changes. If a DSP notices a change in health, mobility or mood, you and the care team hear about it the same day.
Call us before discharge, not after. We can meet the discharge planner's timeline, set up the home, and provide extra hours during the first two weeks when the risk of a return trip is highest. Ask the discharge planner to send us a referral; the checklist above tells them what we need.
Employees. Every DSP passes a Wisconsin caregiver background check before hire, completes competency-based training before the first visit, and is supervised by a Haven Crest Homes supervisor. We carry the insurance and the responsibility.
For program members, we follow the hours authorized in your plan. For private-pay families, visits are typically a minimum of two hours so a DSP has time to do the work well. We'll be upfront if a schedule isn't one we can staff reliably.
You're told as early as possible and offered the named backup who has already met you. We track missed and late visits as a core quality measure because reliability is the whole point.
For program members, usually within a week of authorization. For private pay, often within a few days of the home visit. Hospital discharges can move faster when the discharge planner contacts us early.
Yes. Much of our work is with adults with intellectual and developmental disabilities who want to live in their own homes with support that respects their choices and builds skills rather than dependence.
We're always looking for dependable, compassionate DSPs across Wisconsin, including family caregivers who want to be employed, trained and supported. Email compliance@havencresthome.com with "DSP" in the subject line.

We invest in our DSPs because reliability depends on it: caregiver background checks before hire, competency-based training before the first visit, a supervisor who knows every member, and a real career path so the person you trust stays with you.
Members, families and referral partners are all welcome. We return every inquiry within one business day.
