Errands, appointments, outings and social connection, with a DSP alongside so you keep doing what matters to you.
Groceries, pharmacy, post office, with you or on your behalf as your plan allows.
Accompanying you to medical, dental and other appointments; taking notes you ask us to take; reporting back to your care team.
Clubs, classes, faith services, the library, a walk by the lake. Your interests set the agenda.
Practicing bus routes, budgeting a shopping trip, ordering for yourself, building confidence step by step.
Noticing changes in mood, mobility, appetite, hygiene or the home environment and reporting them promptly.
Help with phone and video calls to family and friends.
Community support is planned around your goals, not a generic activity list. Transportation-related support is provided only where your program authorizes it; we'll be clear about what's covered before the first outing.
DSPs do not make medical decisions, speak for you at appointments unless you ask, or provide transportation outside what your program and our policies allow.
Program members: ask your care manager or IRIS consultant to authorize Haven Crest Homes. Families: ask about hourly private-pay care. See programs and coverage →
One call is enough to find out whether this support is the right fit.
