Respite and transition-home support

Relief for caregivers. Extra hands when coming home.

Scheduled respite so family can rest, and structured support during the first weeks after a hospital, rehab or skilled nursing stay.

What's included

What a visit can include.

Scheduled respite

Regular blocks of time, weekly or monthly, when a trained DSP steps in so the family caregiver can step out.

Transition-home setup

Before discharge, we meet the family and the planner, prepare the home and confirm the schedule so day one is covered.

First two weeks

Increased hours when authorized, during the window when return trips to the hospital are most likely.

Care-team coordination

We follow the discharge instructions within our scope and report changes to the care manager, family and physician.

Routine rebuilding

Help getting back to meals, hygiene, mobility and daily rhythms after a disruption.

Step-down

As things stabilize, hours step down to a sustainable long-term plan.

Who it's for

Who this support fits.

  • Family caregivers who need predictable time off, or a break before burnout
  • Members coming home from a hospital, rehabilitation or skilled nursing facility
  • Discharge planners who need a reliable in-home partner on a tight timeline
  • Families supporting an adult with a disability who want the caregiver to have a life too
What to expect

How we deliver it.

Call before discharge, not after. We can meet a discharge planner's timeline when we're contacted early, and the referral checklist on our main page tells them exactly what we need.

What we don't do

Transition support is non-medical. Skilled nursing, therapy and medication management stay with your home health agency and physician; we coordinate with them, we don't replace them.

Coverage

Covered through Family Care, IRIS or private pay.

Program members: ask your care manager or IRIS consultant to authorize Haven Crest Homes. Families: ask about hourly private-pay care. See programs and coverage →
