Who it's for

Who this support fits.

  • Older adults who need help with one or more activities of daily living
  • Adults with physical or developmental disabilities living in their own home
  • People recovering at home after a hospital or rehabilitation stay
  • Family caregivers who need a trained professional to share the load
What to expect

How we deliver it.

A supervisor visits first to write your Service Plan: which tasks, in what order, with what preferences. Your DSP is trained on that plan before the first visit. Every visit is documented and changes in skin, mobility, appetite or mood are reported to your care team the same day.

What we don't do

Personal care is non-medical. DSPs do not perform wound care, injections, catheter care, clinical assessments or medication administration beyond what your plan allows.

Coverage

Covered through Family Care, IRIS or private pay.

Program members: ask your care manager or IRIS consultant to authorize Haven Crest Homes. Families: ask about hourly private-pay care. See programs and coverage →
